Telemedicine Disclosure

Zind Health LLC provides marketing, technology, and administrative support for access to telehealth services through the FUSE Health Inc. platform. FUSE Health Inc. provides technology and non-clinical administrative services. Clinical care is provided by licensed healthcare professionals through Beluga Health, P.A., or another treating medical practice identified to you before care. This disclosure explains these roles and the limits of receiving care remotely.

Who provides your care

Licensed healthcare professionals make medical decisions independently. Your treating clinician must be authorized to provide care in the state where you are physically located. The treating medical practice and clinician will be identified through your care process. Zind Health LLC does not determine whether a prescription is medically appropriate.

Services are intended for adults age 18 and older who meet the program’s eligibility criteria and are physically located in a supported U.S. jurisdiction. Availability depends on clinician authorization, the treatment requested, and pharmacy fulfillment. Your intake determines current availability. Provide your actual location at each encounter and notify your care team if it changes.

How telehealth works

An evaluation may involve a health questionnaire, secure messaging, telephone, or video, as appropriate for your circumstances and applicable requirements. Your clinician may require a live visit, laboratory tests, medical records, or an in-person examination before recommending treatment.

Telehealth can improve access to care but cannot replace every physical examination or procedure. Technical failures, incomplete information, and delays can affect an evaluation. Your clinician may decline remote treatment or refer you for in-person care. You may ask questions about telehealth and available alternatives before consenting.

Prescriptions and pharmacy services

Completing an intake, enrolling in a program, or paying a fee does not guarantee treatment or a prescription. Prescribing decisions depend on a clinical evaluation. All medications carry risks, and individual results vary. If a medication is prescribed, it is dispensed by a licensed pharmacy authorized to serve your location. The dispensing pharmacy will be identified in prescription or fulfillment communications and on the medication label. Contact information for medication questions will be provided through the care process. Availability, pricing, and delivery depend on the prescribed treatment and pharmacy.

If a compounded medication is offered, it is not FDA-approved. FDA does not review compounded medications for safety, effectiveness, or quality before marketing. A compounded preparation is not the same product as an FDA-approved branded medication.

Privacy and communication

Electronic communications carry privacy and security risks. Review the Zind Health Privacy Policy and the treating medical practice’s Notice of Privacy Practices. Use the patient portal for medical questions and sensitive health information when available. Ordinary email may not be secure and is not monitored for emergencies.

Your clinical team obtains any required telehealth informed consent before care. This website disclosure does not replace that consent or medication-specific counseling.

Emergencies and support

This service is not for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.

For administrative questions, email hello@zindhealth.com. For treatment questions, use the patient portal or communication method supplied by your treating medical practice.
